Replarotomy during the Same Hospitalization and its Prognosis


Abstract
A retrospective survey was made on 80 patients who required relaparotomy because of postoperative complications during the same hospitalization.
1) The number of relaparotomy in 80 patients were 85(2%) among 4,229 patients who had undergone abdominal operations at Hangang Sacred Heart Hospital during the past 5 years from July, 1977 to June, 1982.
2) The sex distribution was male 56 cases(70%) and female 24 cases(30%).
3) The incidences of reoperation according to initial operation were 3.l%(21 cases) in gastric operation, 0.6%(2 cases) in biliary operation, 4%(33 cases) in intestinal operation, 0.4% (7 cases) in appendectomy, 2.8% (3 cases) in splenectomy, 6.9% (6cases) in hepatic operation and 4.6%(13 cases) in exploratory laparotomy.
4) The complications necessitating relaparotomy were peritonitis 21 cases(24.7%), intestinal obstruction 28 cases(32.9%), burst abdomen 19 cases(22.4%), bleeding 13 cases(15.3%) and miscellaneous 4 cases(4.7%).
5) The mean time of occurrence of complications necessitating relaparotomy were 9 days in peritonitis, 9 days in intestinal obstruction, 9 days in burst abdomen and 4 days in bleeding.
6) The mortality rate was 32.5%(26 cases) with 50% in the 6th decades and 11.1% in the second decades.
7) The causes of death were sepsis 14 cases, bleeding 3 cases, cardiopulmonary failure 7 cases and miscellaneous.2 cases.
8) Assessment of the 85 operations indicated that 48 cases(56.5%) had been performed without delay (Group A), 25 cases (29.4%) bad had a short but justifiable delay (Group B), but 12 cases(l4.1%) had had unjustifiably long delay(Group C).
9) The mortality rates of relaparotomy according to the type of complications were 70% in peritonitis, 19.2% in intestinal obstruction, 38.5% in bleeding and 11.1% in burst abdomen.
10) The mortality rates according to the surgical delay were 22.9% in Group A, 36.0% in Group B and 41.7% in Group C.
